Add Decision guardrails for the pull requests section on the ADR Tooling page.

GitHub Action + CLI for automatic ADR surfacing and enforcement in PRs
Inspired by and fully compatible with the adr.github.io workflow
Repo: https://github.com/DecispherHQ/decision-guardian
Matches the exact formatting style of existing tools (ArchUnit, docToolchain, Structurizr)

This follows the CONTRIBUTING guidelines.
